Ministries - Community Health Services
CDM-CHS is a clinic, providing medical and dental care, for Dallas
County residents who are uninsured and for working people without
medical coverage available. To make a medical or dental appointment
please bring the following items in person on a Wednesday or Thursday
between 1:00 pm – 4:00 pm or Friday between 9:00 am – 12:00 pm.

Household Income Info to verify lack of medical coverage resources)

Address (A Bill that
will reflect Current Address to verify Dallas County Residency)

Identification (Valid
Picture ID)

Social Security (To
determine if State/Federal Assistance is available. If you do not
have one, please inform the Receptionist.)
When information is given
to receptionist appointment will be made for a later date. The Dentist
is here only on Monday and Tuesday 9-5pm. There are no emergency
walk-ins with dental.
