TRAC Transition Planning

TRAC coordinates resources and fills the gaps in planning services for youth emancipating from substitute care. If you are a caseworker or care provider or a youth and would like to access TRAC services, please complete the referral form online or download it and fax it to 214.370.9305. Once we receive the information, TRAC staff will contact you within several days, to set up an interview/intake. The intake will be extensive and will last about 2 hours.
